Resumo

Um algorítmo para a detecção e delineação de complexos QRS usando uma única derivação de eletrocardiograma foi desenvolvido e implementado em microcomputador. O sistema, num primeiro estágio, procura efetuar a detecção do QRS e num segundo procede à correção de possíveis falhas na delineação ou de eventuais perdas de complexos. O programa foi desenvolvido em linguagem Assembly usando um conjunto de ar quivos de ECG contendo vários tipos de arritmias. A avaliação do algoritmo foi realizada com registros de 21 pacientes (50000 complexos QRS), obtendo-se 0,42% de falsos-negativos e 0,11% de falsos-positivos. As características e limitações do algorítmo são discutidas.

Abstract

An algorithm for the on-line detection and delineation of QRS complexes in single lead electrocardiograms has been developed and implemented on a microcomputer. Using a sampling rate of 240 Hz the ECG signal is low-pass filtered and the absolute value of its numerical derivative is compared to an adaptative threshould for the detection of the QRS complex. A number of parameters are them transfered to a second stage of the algorithm which delineates the beginning and end of each complex and compensates for any failures of the previous stage. The program has been developed in Assembly language, using the MIT ECG Database as a learing file containing a number of arrhytmias abd artefacts for testing its performance. The algorithm has been evaluated using records of 21 patients (50000 QRS complexes), obtaining 0.42% of false-negatives and 0.11% of false-positives. The characteristics and limitations of the computer algorithm are discussed.
